State-wide Community Health Equity Survey now available

September 15, 2026

The town and state are seeking input on local health needs and community priorities. The state Department of Public Health (DPH) has released the Community Health Equity Survey (CHES), an anonymous survey available in 11 languages, according to an announcement from the town.

According to the town-wide release, CHES invites Massachusetts residents to respond to local health needs, experience and community priorities across the Commonwealth.

The confidential survey can be completed in 15 to 30 minutes, according to the state website. Any resident 12 years or older is invited to answer questions about topics affecting health; including housing, access to health care, mental health, employment, education, substance use, safety, and trusted sources of information.

According to the state government website, this survey gathers information from communities hit hardest by health inequities; these communities include immigrants, older adults, parents and caregivers of children with special health care needs, people of color, veterans, LGBTQ+ individuals, and more.

The survey is expected to be available through the fall until early November. Once the survey closes, results will be shared with community partners, who will use the information to understand where and how to best provide resources to improve health.

The survey is available online here.

Additional information about the survey including its content, origin, privacy, etc are published here.
